Hello there, I would like to know what documents I need.
================
You'll need to provide a proof of citizenship to us.
Have you previously reported your child's U.S. citizenship to us?
No, I haven't.
What if I don't have a proof of citizenship available?
Then we may accept the child's state-issued non-driver's identification card or adoption decree.
And what if I don't have any of these documents?
In that case we will ask to see other documents. Any documents submitted, including the following, must be current not expired and show your name, identifying information date of birth or age and preferably a recent photograph. We could for example ask for an employee identification card.
I see. And what would work as a proof of identity?
For example a U.S. driver's license or a state-issued, non-driver identification card.

Actually, a friend of mine lost their Social Security card. If that happens to me, can I get it replaced somehow?
You can replace your child 's Social Security card for free if it is lost or stolen. However you may not need to get a replacement card, knowing your child's Social Security number is what's important.
